RCF/MR Group Homes Hope Haven owns and operates three RCF/MR group homes - Walton, Gugeler and Northwood. Each 12 bed home is licensed by the Department of Inspections and Appeals providing 24 hour support, supervision and care. All three homes are located in Burlington, in Des Moines County, Iowa on a lovely lake setting. The group homes primarily serve individuals with mental retardation as well as other developmental disabilities. Cottonwood Cottonwood is a 15-bed facility licensed by the Department of Inspections and Appeals under Chapter 57 of the Iowa Administrative Code. It is located in Burlington, Iowa. Hope Haven Development Center is under contract with Des Moines County to manage the operation of the facility beginning July 2007. Cottonwood serves individuals with mental illness and mental retardation, as well as other developmental disabilities. Cottonwood offers long term and transition care and training depending upon each individual's unique needs and abilities. All efforts in the programming realm are oriented toward assuring the least restrictive environment appropriate to address the needs of the individual. Long term care involves continued training in all areas to develop and maintain skills and behaviors so the individual receiving services can live as independently as possible in the least restrictive environment. Services include training in activities of daily living skills, interpersonal and social skills, independent living skills, health services, leisure skills, recreational services, behavior therapy, support services, transitional care, community awareness and transportation services. All services are provided by trained Hope Haven staff or by utilizing consultative services from professionals in the community. ICF/MR Group Homes Brueck and Ertz Home are our ICF/MR group homes. The two eight-bed group homes serve individuals that have a primary diagnoses of mental retardation and must be certified by the Iowa Foundation for Medical Care to be in need of this level of care. We are federally mandated to direct our services towards the acquisition of the behaviors necessary for the individual to function with as much self determination and independence as possible, as well as toward the prevention of loss of skills. We provide 24 hour nursing staff, a one to two, staff-to-client ratio, consultant professionals including occupational therapists, physical therapist, psychologist, dietitian, pharmacy, dental and medical services. Lake View Apartments Lake View Apartments consist of 18 independent living apartments is a Housing and Urban Development (HUD) complex. All rents are adjusted according to income. Individuals residing at Lake View Apartments are considered to be living independently and must be handicapped or disabled according to the Social Security definition. Lake View Apartments are located in Burlington, Iowa, and as the name suggests, has a view of Walton Lake.
